Complete the intake and even pay your balance or copay before coming into the clinic. We know that filling out paperwork is part of what makes visits so lengthy, so we recommend completing the forms a day or two before the appointment. If you forgot to do the forms before the appointment, no worries! Just remember to come in a few minutes early to complete them in clinic.
